Terapias dirigidas en linfomas B: papel de los anticuerpos biespecíficos y CAR-T

Resumen

Los linfomas B constituyen un importante problema de salud pública debido a su elevada incidencia y complejidad terapéutica. En las últimas décadas, el desarrollo de nuevas estrategias de inmunoterapia ha permitido avances significativos en su tratamiento, destacándose los anticuerpos biespecíficos y las células CAR-T como alternativas innovadoras para pacientes con enfermedad refractaria o en recaída. El presente trabajo tuvo como objetivo caracterizar ambas modalidades terapéuticas mediante una revisión bibliográfica actualizada de la literatura científica disponible en español, inglés y portugués. Se analizaron 54 referencias procedentes de bases de datos especializadas, lo que permitió identificar sus principales mecanismos de acción, ventajas y limitaciones clínicas. Los anticuerpos biespecíficos favorecen la activación simultánea de la respuesta inmune frente a antígenos tumorales específicos, destacándose entre los más utilizados el mosunetuzumab, el glofitamab y el epcoritamab. Por su parte, la terapia con células CAR-T se fundamenta en la modificación genética de linfocitos T para reconocer y destruir células malignas, siendo el tisagenlecleucel, el axicabtagene ciloleucel y el lisocabtagene maraleucel algunas de las opciones actualmente disponibles. Aunque la terapia CAR-T suele mostrar una mayor efectividad clínica, también se asocia con una mayor toxicidad y una logística de aplicación más compleja. En conjunto, ambas estrategias representan una transformación en el abordaje de los linfomas B al ofrecer tratamientos más específicos y potencialmente menos tóxicos que las terapias convencionales, consolidándose como pilares fundamentales de la inmunoterapia moderna.
