Salpingectomía bilateral como prevención del cáncer de ovario

Resumen

El cáncer epitelial de ovario, especialmente el carcinoma seroso de alto grado, continúa siendo una de las neoplasias ginecológicas más letales debido a su diagnóstico habitualmente tardío. Evidencias recientes han demostrado que este subtipo tumoral se origina predominantemente en las fimbrias de las trompas de Falopio, lo que ha impulsado la propuesta de la salpingectomía profiláctica como medida preventiva, incluso en mujeres sin factores de riesgo genético conocidos. Metodología: Se realizó una revisión analítica de literatura científica publicada entre 2020 y 2024 en bases de datos indexadas como PubMed, Scopus y Springer, enfocándose en estudios observacionales y de cohorte que evaluaron la relación entre la salpingectomía profiláctica y la reducción de incidencia del cáncer de ovario. Se incluyeron artículos que aportaran resultados poblacionales con sustento estadístico y aplicabilidad clínica. Resultados: Los estudios revisados coinciden en una significativa reducción del riesgo de carcinoma seroso de alto grado en mujeres sometidas a salpingectomía profiláctica, especialmente cuando se realiza en el contexto de cirugías ginecológicas electivas. Algunos trabajos incluso reportan una incidencia nula de esta neoplasia tras más de una década de seguimiento. Discusión: La evidencia actual refuerza el valor preventivo de la salpingectomía, tanto en portadoras de mutaciones BRCA como en mujeres de riesgo promedio. La estrategia se perfila como un enfoque quirúrgico seguro y éticamente viable, con importantes implicaciones para la salud pública. Conclusión: A partir del análisis crítico de la literatura, se concluye que esta intervención debe considerarse en cirugías ginecológicas electivas. Su incorporación en protocolos quirúrgicos preventivos puede traducirse en una reducción sustancial de la carga oncológica femenina en los próximos años, particularmente si se promueve desde la atención ginecológica primaria.
